This post will answer your queries regarding documents needed to be submitted from the employee and also for the nominee or family members what documents need to be submitted after the death of the pf account holder to claim his/her monthly pension by filling Form 10D.

EPF Form 10D – EPS95

Whenever an employee is registered under Employee Provident Fund Organization (EPFO), automatically he/she will be enrolled as a member of Employee Provident Fund  (EPF) and Employee’s Pension Scheme (EPS). The EPFO has revised and made certain changes in the EPS 1995 para which is called EPS95. The employee’s joined on or after the year 1995 will be benefited according to the EPS95 pension scheme.

Types of Pension Claimed

  1. Superannuation Pension: This type of pension can be claimed by the pf account holder after retirement at the age of 58 years.
  2. Reduced Pension: This is an optional pension for the employees which can be claimed with a discounted rate of 4% after 50 years of age.
  3. Disablement Pension: It’s an early pension claimed monthly due to permanent and total disablement.
  4. Widow & Children Pension: Monthly pension benefit for the widow and children after the employee’s death.
  5. Orphan Pension: Such monthly pension is availed by the surviving sons/daughters of the deceased employee up to their 25 years of age.
  6. Nominee Pension: Monthly pension benefit goes to the employee’s nominated member after the death of the employee in case no family members exist.
  7. Dependant Parent: In this case monthly pension goes to the dependent parents if the member’s status is unmarried during the death.

Case 1: Documents for Pension Scheme Certificate

  • In this case, the employee has completed at least 10 years of service and claiming a pension scheme certificate. Here are the documents to be submitted.
  1. The original Form 10C (signed and seal attested by the employer).
  2. ID Proofs like Pan Card, Aadhaar Card, other (Member Name, Father Name, Date of Birth should be same in all the id proofs)
  3. Employee’s birth certificate/10th Marks card copy to confirm the Age-proof.
  4. Nominee ID proof (Aadhaar card, Pan Card, and Voter ID)
  5. If an employee’s children are below 25 years of age, then their birth certificate is required.
  6. Scheme Certificate (if an employee has a previous employer’s scheme certificate)
  7. Aadhaar linked the Mobile number of the pf account holder.
  8. Cancelled cheque of a bank.

Case 2: Documents for claiming Monthly Pension

  • In this case, the employee has taken retirement after the age of 58 years and the total service count is of more than 10 years and claiming a monthly pension benefit. Here are the documents to be submitted.
  1. The original Form 10D (3 set originals signed by the employee)
  2. ID Proofs like Pan Card, Aadhaar Card, other (Member Name, Father Name, Date of Birth should be same in all the id proofs)
  3. Employee’s birth certificate/10th Marks card copy to confirm the Age-proof.
  4. Nominee ID proof (Aadhaar card, Pan Card, and Voter ID)
  5. If an employee’s children are below 25 years of age, then their birth certificate is required.
  6. No pension certificate (declaration pension not getting)
  7. Scheme Certificate (if an employee has a previous employer’s scheme certificate)
  8. Joint family photograph (3 colour passport size photos)
  9. Employee aadhaar linked mobile number, height and other identification marks.
  10. Original cheque of a Stae Bank/Punjab National Bank. (list of disbursing banks)

Case 3: Documents for Death Pension Claim (Married Employee)

  • In this case, due to the married employee’s death, the wife and children become eligible to claim a monthly pension. Here are the documents to be submitted.
  1. Form 20 (1 set) and original Form 10D (3 set originals signed by the employee’s wife/nominee).
  2. Employee’s id proofs like Pan Card, Aadhaar Card, Driving Licence, and other id proof.
  3. Original Death certificate of an employee.
  4. Wife/Nominee ID proof (Aadhaar card, Pan Card, and Voter ID)
  5. If an employee’s children are below 25 years of age, then their birth certificate is required.
  6. Affidavit Copy (Pension can only be claimed by the Wife/Children of the claimant in case of death. Wife/Children needs to declare their dependency on employee stating that they are not getting any kind of monetary benefit from any other department and also need to share the work profile of employee’s last organization )
  7. Joint family photograph (3 colour passport size photos)
  8. Nominee’s aadhaar linked mobile number, height and other identification marks.
  9. Original cancelled cheque of a State Bank/Punjab National Bank. (list of disbursing banks). All family’s member account should be in the same branch.

Case 4: Documents for Death Pension Claim (Unmarried Employee)

  • In this case, due to the unmarried employee’s death, the father and mother become eligible to claim a monthly pension. Here are the documents to be submitted.
  1. Form 20 (1 set) and original Form 10D (3 set originals signed by the employee’s Father/nominee, not Mother).
  2. Employee’s id proofs like Pan Card, Aadhaar Card, Driving Licence, and other id proof.
  3. Original Death certificate of an employee.
  4. Father and Mother/Nominee ID proof (Aadhaar card, Pan Card, and Voter ID)
  5. Birth Certificate.
  6. Affidavit Copy (Pension can only be claimed by the Father and Mother of the claimant in case of death. Father and Mother need to declare their dependency on employee stating that they are not getting any kind of monetary benefit from any other department and before joining at organization employee was what to do.)
  7. Joint family photograph (3 colour passport size photos)
  8. Nominee’s aadhaar linked mobile number, height and other identification marks.
  9. Original cancelled cheque of a State Bank/Punjab National Bank. (list of disbursing banks). All family’s member account should be in the same branch.
